Working with Pathways to Independence supporting homeless people

Recovery Action Plan sessions are available for homeless individuals that require 1 to 1 assistance in recovering their mental health and wellbeing so that they can progress on the journey to independent sustained living.
The Recovery Action Plan (RAP) consists of 6-12 weekly 45 minute sessions with a Mind Wellbeing Worker looking at 10 key areas of life and with their support setting goals and taking action to achieve better wellbeing. The 10 key areas are:

  • Managing Mental Health
  • Life Skills
  • Physical Health and Self-care
  • Aspirations
  • Relationships
  • Social Networks
  • Employment/Volunteering/Training
  • Identity & Self-esteem
  • Addictive/Challenging Behaviour
  • Responsibilities
